There are two major steps to a TAHSN Board of Record review and to obtaining approval to conduct a study at an institution:
1. The first step is completing the TAHSN Board of Record Application including the Site-specific Form(s)
2. If the initial application is approved, the second step is submitting to the Lead REB and completing the TAHSN Board of Record Study Agreement(s). If you are adding one or multiple site(s) to an existing REB-approved study, please contact the Lead REB's Site Navigator for information on how to proceed and obtain approval for the new site(s).
